The City of Hopewell contracts with the following health care providers to maintain the health care needs of City employees. Should you require additional information concerning health, dental, or the employee assistance programs, please contact the health care provider or the Human Resources Office for further assistance.

Southern Health Services, Inc.
Employees have the option of participating in the low option Virginia Value Health Maintenance Organization, the high option Virginia Care Health Maintenance Organization, or the Point of Service Health Care Plan.  The HMO Plans are non-referral based programs that consist of a network of physicians, specialists, hospitals and other health care providers and services in Virginia.  With Southern Health’s HMO coverage, you may select a family practice physician, general practice physician, internist, pediatrician, or an OB/GYN as your Primary Care Physician.  Each family member may select a different physician to be his or her Primary Care Physician.  Your PCP is a great resource for coordinating your health care.  With Southern Health’s POS coverage, you have the option to receive care from a participating network of providers and/or seek care at a reduced level of benefits from nonparticipating providers.

Delta Dental
Delta Dental Plan of Virginia (DDPV) offers an alternative way to provide for you and your family's dental care needs - economically and conveniently through the Delta Care program. This program was founded on the principle of delivering quality dental care and preventing dental problems before they start. DDPV has contracted with a network of private dental offices. As an enrollee in the Delta Care program you select from any of these dental offices for your needs. The Delta Care program provides all reasonable and customary dental care if care is rendered by your assigned Delta Care panel dentist. There is no cost for covered services to the primary enrollee or eligible dependent enrollee except for copayments on certain procedures.

REACH
REACH is a confidential Employee Assistance Program (EAP) to help employees resolve job-related, personal and family problems. The EAP can help you and your eligible family members deal with many situations, including the following: stress, marital and family problems, emotional problems, addictive behaviors, drug and alcohol abuse, job-related problems, and legal and financial referrals. The EAP provides you and your eligible family members with assessments and short-term problem resolution at no cost to you. Most requests for assistance will be initiated by an employee or family member directly by calling the EAP assistance phone line 24 hours a day - 7 days a week.
