The City of Hopewell contracts with the following health care providers to maintain the health care needs of City employees. Should you require additional information concerning health, dental, vision, or the employee assistance programs, please contact the individual provider or the Human Resources Office for further assistance.

United Healthcare
Employees have the option of participating in the  Low Option Choice Plan 049M Health Maintenance Organization, the High Option Choice Plan 044M Health Maintenance Organization, or the Point of Service Health Care Choice Plan 097M.  The HMO Plans are non-referral based programs that consist of a network of physicians, specialists, hospitals and other health care providers and services in Virginia.  With United Healthcare's HMO coverage, you may select a family practice physician, general practice physician, internist, pediatrician, or an OB/GYN as your Primary Care Physician.  Each family member may select a different physician to be his or her Primary Care Physician.  Your PCP is a great resource for coordinating your health care.  With United Healthcare's POS coverage, you have the option to receive care from a participating network of providers or seek care at a reduced level of benefits from nonparticipating providers.

Delta Dental
Delta Dental Plan of Virginia (DDPV) offers an alternative way to provide for you and your family's dental care needs - economically and conveniently through the options offer in the City's Delta Dental Program.  With our program, you may enroll in the PPO Plus Premier High Option or enroll in the PPO Plus Premier Low Option depending on your particular dental needs.  Our program is design to make covered benefits more affordable.  In most cases, the plan will pay a portion of the cost of your covered benefit (up to the plan's maximum).  You may be responsible for deductibles, coinsurance and in some cases, dental charges that exceed what Delta Dental covers.

REACH
REACH is a confidential Employee Assistance Program (EAP) to help employees resolve job-related, personal and family problems. The EAP can help you and your eligible family members deal with many situations, including the following: stress, marital and family problems, emotional problems, addictive behaviors, drug and alcohol abuse, job-related problems, and legal and financial referrals. The EAP provides you and your eligible family members with assessments and short-term problem resolution at no cost to you. Most requests for assistance will be initiated by an employee or family member directly by calling the EAP assistance phone line 24 hours a day - 7 days a week.
